TARGETED CASE MANAGEMENT

Targeted Case Management services are defined as those services which assist the individuals in gaining access to medical, social, educational and other needed servivces.

Targeted Case Management includes the following elements of services:

Assessment of an eligible individual to determine service needs by:

taking the individual's history,
identifying the individual's needs and completing the related documentation, and
gathering information, if necessary, from other sources such as family members, meidcal providers, social workers, and educators, to form a complete assessment of the individual

Development of a specific support/care plan that:

is based on the information collected through the assessment,
specifies the goals and actions to address the medical, social, educations, and other service needs of the individual,
includes activities such as ensuring the active participation of the eligible individual, and working with the individual (or the individual's legal representative) and others to develop such goals, and identify a course of action to respond to the assessed needs of the eligible individual.

Referral and related activities:

to help an individual obtain needed services including,
activities that help link the individual with medical, social, education providers, or other programs and services that are capable of providing needed services and scheduling appointments for the individual

Monitoring and follow-up activities including:

activities and contacts that are necessary to ensure the care plan is implemented and adequately addreses the individual's needs, and which may be with the individual, family members, providers, or other entities and conducted as frequently as necessary to determine:
1. whether services are being furnished in accordance with the individual's care plan;
2. whether the services in the care plan are adequate; and whether there are changes in the needs or status of the individual, and if so, making necessary adjustments in the care plan and service arrangements with the providers

TARGETED CASE MANAGERS AND SUPPORT STAFF

Targeted Case Manager - Mary Gann began working at Twin Rivers in July 2010. She has worked in the Special needs area for 14 years. She recently moved back to Kansas from Southeast Texas where she was employed by the State of Texas working with individuals with special needs. She loves to be around people and to stand up for rights of other individuals.

Targeted Case Manager - Chad Irwin began working at Twin Rivers in July 2007. He has several years hands on experience working with the disabled as a para at a local school.

Targeted Case Manager - Sandy Bogle began working at Twin Rivers in August 2009 as a vocational trainer and became a Targeted Case Manager in August 2011. She spent 7 years working as a para-professional at Winfield State Hospital/Serendipity before it closed and 4 years as a para-professional at Winfield High School. Sandy has enjoyed working direct care with the individuals and looks forward to advocating for them in a different way.

Support Services Secretary - Catheryn Widner has been employed at Twin Rivers since November 1996. She is in charge of maintaining all of the individual's records as well as performing a variety of duties for all departments. She keeps the department organized and running smoothly. She also fills in as Targeted Case Manager whenever necessary. She is one of the individuals responsible for screen printing artwork and new orders.

Certified Medication Aid - Carol Logan has been employed at Twin Rivers since March 1998. She previously assisted with training individuals in the Facility Services Program. She currently assists individuals with medical appointments and advocates for their medical treatment. Carol also serves as liaison between physicians and program staff.

Registered Nurse - Colleen Detar Colleen Detar began working for Twin Rivers in January 2011 as the agency Registered Nurse. As the agency Nurse she serves as an advocate for the consumers medical needs. She is responsible for teaching medication administration training to staff and oversees the medication administration. In addition she completes wellness monitoring and tends to any medical needs of the consumers including communicating with physicians. These are just a few of her job duties. Colleen graduated from Butler Community College’s RN program in December 2010 with honors. She decided to become a nurse to help individuals make healthy choices and maximize their quality of life. Her favorite aspect of working at Twin Rivers is functioning as an advocate for the consumers. She especially enjoys the close knit teamwork environment and the cooperativeness of the healthcare professionals in the area.
