SERVICE COORDINATION

Service Coordination assists the person and their support network to identify, select, obtain, coordinate, and use both paid services and natural supports as may be available to enhance the person's independence, integration, and productivity consistent with the person's capabilities and preferences as outlined in the individual's person-centered support plan.
Service Coordination includes the following elements of services:

SERVICE COORDINATORS AND SUPPORT STAFF


Service Coordinator - Mary Lowe began working at Twin Rivers in July 2010. She has worked in the Special needs area for 14 years. She recently moved back to Kansas from Southeast Texas where she was employed by the State of Texas working with individuals with special needs. She loves to be around people and to stand up for rights of other individuals.

Service Coordinator - Chad Irwin began working at Twin Rivers in July 2007. He has several years hands on experience working with the disabled as a para at a local school.

Service Coordinator - Pat Gray began working at Twin Rivers in February 2008. She has over 30 years of experience working with disabled individuals. She has most recently worked as a residential manager and has also performed direct care.

Support Services Secretary - Catheryn Widner has been employed at Twin Rivers since November 1996. She is in charge of maintaining all of the individual's records as well as performing a variety of duties for all departments. She keeps the department organized and running smoothly. She also fills in as Service Coordinator whenever necessary. She is one of the individuals responsible for screen printing artwork and new orders.

Certified Medication Aid - Carol Logan has been employed at Twin Rivers since March 1998. She previously assisted with training individuals in the Facility Services Program. She currently assists individuals with medical appointments and advocates for their medical treatment. Carol also serves as liaison between physicians and program staff.

Registered Nurse - Colleen Detar Colleen Detar began working for Twin Rivers in January 2011 as the agency Registered Nurse. As the agency Nurse she serves as an advocate for the consumers medical needs. She is responsible for teaching medication administration training to staff and oversees the medication administration. In addition she completes wellness monitoring and tends to any medical needs of the consumers including communicating with physicians. These are just a few of her job duties. Colleen graduated from Butler Community College’s RN program in December 2010 with honors. She decided to become a nurse to help individuals make healthy choices and maximize their quality of life. Her favorite aspect of working at Twin Rivers is functioning as an advocate for the consumers. She especially enjoys the close knit teamwork environment and the cooperativeness of the healthcare professionals in the area.
